Denial Help
Analyse denial reasons, understand CARC and RARC codes, and generate appeal letters with AI.
Documentation draft — not a verified product specification. The four-stage workflow labels and export formats below carry the source document's verify notes. Do not treat them as confirmed until checked against the live UI.
Overview
Denial Help supports research into claim adjustment/denial reasons and helps users organise a structured investigation, identify possible causes, review next steps and prepare draft appeal materials for professional review.
Who should use it
Denial specialists, billers, revenue-cycle teams and billing supervisors.
Prerequisites
A denial to analyse, handled through your organisation's approved process. Use synthetic, non-identifying information only. See Data and Privacy.
Supported concepts
- CARC: explains why a claim or service was adjusted.
- RARC: supplies additional explanation about an adjustment or remittance.
- Payer context: insurer or payer name and relevant non-PHI details.
- Procedure and modifier context: helps frame investigation when supported.
- Denial narrative: only generic, synthetic or properly non-identifying text.
Accuracy note: CO-50 relates to medical necessity/coverage criteria in its standard CARC context. Do not label it as a bundling denial. Confirm the exact official code description and current code-list source before publishing any other code description.
Step-by-step instructions — four-stage workflow (verify labels against live UI)
Stage 1 — Claim Details
Enter permitted structured information such as payer, adjustment code, procedure, modifier and other supported non-PHI fields. Check for required inputs and code validity.
Stage 2 — Clinical Context
Provide generalised, non-identifying context only. Do not enter identifiable patient history, diagnoses tied to a specific individual, chart extracts, dates of birth or member numbers. The purpose is to frame a synthetic scenario, not process real clinical records.
Stage 3 — Review & Edit
Review the generated denial analysis, potential reasons, evidence checklist, suggested actions and any appeal-letter preview. Edit or remove unsupported claims before proceeding. Verify policy citations and time-sensitive appeal requirements with the payer.
Stage 4 — Export & Finalize
Where currently implemented, export a human-reviewed draft in supported formats. Prior product discussions describe PDF, DOCX and text options — confirm these exact formats and whether they work in the deployed build before treating them as guaranteed. They are described in product discussions only.
AI Denial Analysis
The AI may help organise:
- Meaning of the denial/adjustment code.
- Potential causes worth checking.
- Missing or conflicting documentation questions.
- Payer-policy research needs.
- Suggested follow-up checklist.
- Draft appeal language for human review.
Outputs should clearly separate known code meaning, source-supported policy and AI hypotheses. The model must not claim a specific root cause without evidence.
Appeal-letter drafting
A draft can provide a professional structure: subject, general denial reference, reason for reconsideration, supporting evidence checklist and request for review. Use fictional placeholders, not real patient data, in Release 1.0. Staff must validate payer instructions, timely-filing limits, required attachments and legal language independently.
Do not describe the tool as automatically submitting appeals. It does not autonomously overturn denials, contact payers, submit appeals or guarantee reimbursement.
Example scenario (synthetic / non-PHI only)
A denial specialist researches a synthetic CO-50 adjustment, selects the payer, and reviews possible medical-necessity documentation and policy questions. The specialist confirms the payer's actual coverage policy and edits a draft appeal outline. The final decision and any submission occur in authorised external systems.
Limitations
Automated appeal submission, AI payer-call assistance, follow-up tracking, deadline reminders, payer playbooks, denial-trend dashboards and real claim ingestion are not confirmed as shipped. See the Roadmap.
Troubleshooting and safeguards
- Missing CARC/RARC: verify the exact code and current official definitions.
- Generic answer: add allowed non-identifying context and review sources.
- Incorrect payer assumption: select the correct payer and applicable policy jurisdiction.
- Export issue: retry or use the supported text/copy workflow, if available.
- Hallucinated citation: do not use it; report and verify independently.
- The letter needs correction: edit it before sending; do not send a draft you have not checked.
